We want to inform you of a notice that will be sent to your personal email. This notice is a DocuSign and will require your signature. This signed document will allow our new partner to service your medical policy. PLEASE SIGN THIS UPON RECEIVING. Again, this document is to provide our new partner access to service your medical policy only, no changes will be made to your policy.
