Understanding your benefitbay® Documents

You may be required to provide your ICHRA notice or other qualifying life event (QLE) documents to your insurance carrier - Documents is where you can start.

Select the "Documents" tab on the left navigation bar to review any documents you signed during your enrollment journey or are provided as part of your ICHRA experience.

Individual Coverage HRA Notice (ICHRA Notice)

If your insurance carrier contacts you requesting proof of  Qualifying Life Event, this is the document you would provide. Most carriers require proof that an individual is eligible for a change of coverage outside of the national enrollment period. If able it will be provided during application submission, however, carriers may request it directly from you as well. 

Please provide promptly if requested by the carrier. If not provided, the carrier may reject your application and you will be at risk for no medical insurance. 


Medical Plan Summary of Benefits

This document will show you a more detailed view of your medical plan benefits as provided by the insurance company

It is important to view the documents provided directly inside your insurance carrier member account for a full set of current benefits.  Benefitbay is not an insurance carrier and cannot guarantee the benefits offered by your insurance carrier.


Signed HIPPA Agreement

This was a document signed during your enrollment journey, before shopping for a plan. In general, it provides benefitbay the ability to advocate with carriers on your behalf. Since these are individual policies, some carriers will still require communication directly from you. 


Signed ICHRA Agreement

This document was signed during your enrollment journey, before shopping for a plan. In general, it outlines the benefits your employer is providing and the explanation of how your benefit is processed.
